Send UMP Your National Provider Identifier (NPI) Number
The National Provider Identifier (NPI) is a standard unique identifier that all providers are required to submit on claims. Providers must submit their NPI numbers directly to UMP so that we can match your new identifier with existing identifiers in our system.
For Smaller Practices:
- If you are submitting one or two NPI numbers, either type 1 (individual)
or type 2 (organization), use our
online
Provider Update form. If you prefer, you can
download a printed form
(194K) and fax
(206-521-2001) or mail it to UMP.
For Larger Practices or Health Care Facilities:
- If you are submitting several type 1 NPI numbers for providers in your practice, download the sample type 1 NPI spreadsheet, include the necessary information, and email it to UMP Provider Relations at umpprovider@hca.wa.gov.
- If you need to submit a type 2 (organization) NPI with multiple sub-parts, please call the UMP Provider Relations Department at 1-800-292-8092. They will work with you to get the needed information.
