Send UMP Your National Provider Identifier (NPI) Number
The National Provider Identifier (NPI) is a standard unique identifier
that all providers are required to submit on claims. Providers must submit their NPI numbers directly to UMP so that we can
match your new identifier with existing identifiers in our system.
For Smaller Practices:
For Larger Practices or Health Care Facilities:
- If you are submitting several type 1 NPI numbers for providers in
your practice, download the sample
type 1 NPI spreadsheet, include the
necessary information, and email it to UMP Provider Relations at
umpprovider@hca.wa.gov.
- If you need to submit a type 2 (organization) NPI with multiple sub-parts,
please call the UMP Provider Relations Department at 1-800-292-8092.
They will work with you to get the needed information.
