The ActivePHR^SM program from ActiveHealth Management gives you a secure, online Personal Health Record (PHR) for every member of your family* enrolled in UMP.

Your ActivePHR is private and confidential. You control whether anyone else can see the information in your ActivePHR.

How It Works

ActivePHR lets you keep track of all of your doctor visits, health screenings, and prescription drug history. The information is updated automatically every time you receive services covered by UMP. You can also add information that only you would know, like any over-the-counter medications or vitamin supplements you take.

Here's How ActivePHR Can Help You Manage Your Health Information:

Lets you print out information, like a list of your child's immunizations or an emergency wallet card.
Lets you create email reminders about preventive care screenings or other exams and tests.
Makes it easy to share information with your doctors. You can print out a health summary for your doctor – or even let your doctor see your ActivePHR online.
Compares your health information against the latest medical knowledge. If it finds anything you should know about, it sends you an alert so you can use it to talk with your doctor.

* Please note: Dependents under age 18 cannot access ActivePHR directly, but their parent/guardian can maintain a personal health record for them.
