Nurses are available Monday through Friday 6:30 am to 8 pm Pacific time, and Saturday 8 am to 1 pm Pacific time.
Enrollees who have diabetes, congestive heart failure, or coronary artery disease can work one-on-one with a registered nurse over the phone through a program called Informed Care Management (ICM) from ActiveHealth Management.
ICM is private and confidential, and there's no cost to eligible enrollees* to participate. If you are eligible and choose to participate, you may also receive a $30 Health Counts bonus award from UMP.
How It Works
ICM pairs you with a nurse who will be your health coach. Your nurse will put together a personalized plan for you and work with you every step of the way. The goal is to help you successfully manage your medical condition.
You can talk to your nurse over the phone as often as you like. Your nurse can:
- Help you better understand your condition
- Help you follow your doctor's instructions
- Answer questions about medications
- Explain lab and test results
- Help you find ways to exercise that work for you
- Send you helpful information in the mail
ICM does not take the place of your doctor. It is, however, a great resource that can help you and your doctors better manage your condition.
* Please note: Informed Care Management is not available to enrollees under the age of 18 or retirees covered by Medicare.
