New Health Counts Bonus Awards
UMP has introduced more ways to reward yourself for good health habits in 2009. Now adult enrollees 18 and over can earn a $30 gift card bonus award in addition to the $30 gift card for taking the Health Counts health assessment.
Three Ways to Earn a Bonus Award
Here's how you earn your $30 bonus:
- You have diabetes or a heart condition and you participate in the Informed Care Management program.
- You smoke or chew tobacco and you participate in the Free & Clear tobacco cessation program.
- You exercise enough to meet the federal government's minimum physical activity guidelines for adults and you report your exercise habits when you take the health assessment.
You can earn only one $30 bonus reward, in addition to the $30 gift card for doing the health assessment. If you qualify for the Informed Care Management or Free & Clear programs, you need to participate in the recommended program to earn a bonus. You cannot receive the bonus for exercising, even if you qualify.
Redeeming Your Bonus Gift Card
Your $30 bonus award arrives the same way as your gift card for taking the health assessment. Hallmark Insights* will send you an email with the subject line, "Uniform Medical Plan has sent you a Health Counts Award," and a link to your reward. Just click on the link and you'll get a list of more than 350 merchants to choose from. The list includes Macy's, Best Buy, Schuck's, Target and many others. You can also select a gift card to use at select movie theaters and restaurants.
