When you take the Health Counts health assessment, Uniform Medical Plan will send you a $30 gift card you can use at more than 350 stores, restaurants and entertainment venues plus you may also earn a $30 bonus! You'll also receive a personalized report showing your health status and your potential risk for conditions such as heart disease, cancer or diabetes.
How It Works
The health assessment takes about 20 minutes and asks about your diet, exercise and preventive care habits. After you answer all the questions, you'll receive your gift card by email within 14 days from Hallmark Insights*. If your gift card doesn't arrive within that time, check your junk mail or spam folders. To make sure your card arrives in your inbox, add HallmarkInsights@hallmarkinsights.com to your email contact list. Redeem your card by clicking on the link in the email.
Earn a Health Counts $30 Bonus Award
Health Counts now includes $30 gift card bonus awards for members who make healthy choices. Here's how you earn your $30 bonus:
- You have diabetes or a heart condition and you participate in the Informed Care Management program.
- You smoke or chew tobacco and you participate in the Free & Clear tobacco cessation program.
- You exercise enough to meet the federal government's minimum physical activity guidelines for adults and you report your exercise habits when you take the health assessment.
You can earn only one $30 bonus reward, in addition to the $30 gift card for doing the health assessment. If you qualify for the Informed Care Management or Free & Clear programs, you need to participate in the recommended program to earn a bonus. You cannot receive the bonus for exercising, even if you qualify.
Two Ways to Access the Health Assessment
The health assessment is available online through ActivePHR, a secure personal health record where your answers to the health assessment will be stored. You can get started one of two ways:

--Log in to your UMP Medical Account and select "Health Assessment." If you need to open an account, select "register now."
Your Privacy is Protected
The information you enter into your health assessment is protected. The health assessment and ActivePHR personal health record are provided by ActiveHealth Management, a third-party vendor that adheres to privacy guidelines, so no one else has access to your personal data or results.
* Hallmark Insights is a third-party vendor contracted to issue Health Counts gift cards. The company does not have access to your health information and will not share, sell or publish your email address.
