When you take the UMP Health Counts eHealth Survey, your healthy behaviors help earn you a $30 gift card, plus the following benefits:

Important personalized wellness information that can help you lower your risk of disease.
Reminders about Uniform Medical Plan's preventive care benefits.
A scorecard to track your progress toward staying fit and healthy year to year.

How It Works

You get 40 points just for completing the survey, and earn more points based on your answers about your exercise habits, diet and preventive care screenings.

If your answers add up to 100 points, you don't need to do anything else.You'll receive your $30 gift card by email between five and 10 days from Hallmark Insights.* If your gift card doesn't arrive within that time, check your junk mail or spam folders, or add HallmarkInsights@hallmarkinsights.com to your contact list to make sure the email goes to your inbox when it arrives.

If you don't earn 100 points with the survey, go to your Health Counts scorecard to check off more healthy behaviors and earn more points throughout the year. Non-smokers need to earn a minimum number of points in two categories. Tobacco users need only take the eHealth Survey and participate in Free & Clear, UMP's free tobacco cessation program, to earn the gift card. How to Take the eHealth Survey

Completing the eHealth Survey takes about 20 minutes. Here are the steps to get you started:

Click "Log in to Health Counts and take the eHealth Survey."
If you don't already have a secure online medical account, select "register now" and follow the prompts.
On the Member Welcome page, click "eHealth Survey."

How to Earn Your Health Counts Points

Exercise at least 20 minutes five days per week.
Eat five or more half-cup servings of fruits and vegetables most days.
Get preventive care screenings such as an annual check-up.
Register for Free & Clear if you smoke or chew tobacco. (Tobacco users must register with Free & Clear to earn the $30 gift card.)

* Hallmark Insights is a third-party vendor contracted to issue Health Counts gift cards. The company does not have access to your health information and will not share, sell or publish your email address.
